We deliver a range of services designed to support and empower some of the most vulnerable groups across Kent, Medway, East Sussex and London. We have innovative projects working with; children and young people, young carers, unemployed young people, people living with a disability, adult carers, older people, socially isolated people, mental health awareness for young people and community transport.
Our services include:
Kent Young Carers, East Sussex Young Carers and London Young Carers support for young people who are responsible for caring for a parent or family member who has a disability, mental health issue or substance abuse problem.
Short Breaks for disabled children service across Kent providing opportunities for disabled children & young people to have enjoyable social experiences as well as offering a valuable break for their parents or carers
Safe an innovative youth-led mental health & wellbeing project delivered in Kent and the London Borough of Greenwich.
Dial 2 Drive community transport scheme for older or less mobile people living in West Kent to be taken to appointments by volunteer drivers.
Wellbeing Navigator Service in Medway offering people free impartial advice, support and guidance to remain independent in their own home and reduce hospital re-admissions.
Wellbeing Matters Navigation & Adult Carer Service in Dartford, Gravesham, Swanley and Swale is a dual service supporting adult carers and adults with complex needs.
Social Prescribing connects people with appropriate community resources and support services, with the aim of enabling people to increase their independence and ability to self-manage, thereby reducing demand on health services .
CAMEO (Come and Meet Each Other) working with people over 50, living in Dartford, Gravesham and Swanley, who are socially isolated or experiencing loneliness due to changing life circumstances.
